Declaration of Identity

In order to complete this correction, two (2) photocopies of a birth certificate, a photocopy of the applicant's ID and/or two (2) copies of any form of documentation with the other name that is not present on the birth certificate.  There must also be one witness who knows the individual requesting the identity declaration for at least 10 years or more and can vouch for the validity and legal use of the name in question.
